Evaluation of left atrial volume and functional changes in patients with diabetic nephropathy using real-time three-dimensional speckle tracking imaging

Abstract
Objective:To investigate the application value of real-time three-dimensional speckle tracking imaging(RT3D-STI)in evaluating changes of left atrial volume and function in patients with diabetic nephropathy(DN).Method:A total of 131 patients with diabetic nephropathy(DN group)treated at Nantong Third People's Hospital from January 2022 to September 2024 were selected as the study subjects.Additionally,97 patients with type 2 diabetes mellitus(T2DM group)and 97 healthy volunteers(control group)during the same period were included.The patients in DN group were divided into early group(n=69)and middle-late group(n=62)according to Mogenson stage.Conventional parameters were measured using two-dimensional echocardiography.Left atrial structural parameters,functional parameters,and strain parameters were obtained using RT3D-STI.Results:There were no statistically significant differences in conventional parameters such as left atrial diameter(LAD),left ventricular ejection fraction(LVEF),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),and the ratio of early diastolic mitral inflow velocity(E)to late diastolic mitral inflow velocity(A)among the three groups(P>0.05).Comparisons of left atrial structural parameters,including maximum left atrial volume(LAVmax),minimum left atrial volume(LAVmin),and pre-contraction left atrial volume(LAVpre),showed DN group>T2DM group>control group(P<0.05).Comparisons of functional parameters,including left atrial ejection fraction(LAEF),left atrial active ejection fraction(LAaEF),and left atrial passive ejection fraction(LApEF),showed DN group<T2DM group<control group(P<0.05).Comparisons of strain parameters,including left atrial reservoir strain(LASr),left atrial conduit strain(LAScd),and left atrial contractile strain(LASct),showed DN group<T2DM group<control group(P<0.05).The levels of LAVmax,LAVmin,and LAVpre in the middle-to-late stage group were higher than those in the early-stage group(P<0.05),while the levels of LAEF,LAaEF,LApEF,LASr,LAScd,LASct,and left atrial reservoir circumferential strain(LASr-c)were lower than those in the early-stage group(P<0.05).Conclusion:RT3D-STI can detect changes in left atrial volume and function in DN patients earlier than conventional two-dimensional echocardiography.In DN patients,left atrial volume significantly increases,left atrial function is altered,and strain values decrease.关键词
