Abstract

Objective:To explore the predictive value of echocardiography for assessing the severity of coronary artery stenosis by the thickness of epicardial fat anterior to the right ventricular(RV)free wall,pericardial fat thickness,left atrioventricular groove epicardial fat thickness,and aortic sinus(AS)angle.Methods:From May 2023 to August 2024,patients who underwent coronary angiography at Weifang People's Hospital were selected.The combined predictive values of pericardial fat and AS angle were analyzed for the degree of coronary artery stenosis.Results:Based on the degree of vascular stenosis on coronary angiography,129 patients were divided into a non-coronary artery disease(non-CAD)group(n=33)and a coronary artery disease(CAD)group(n=96).The anterior epicardial fat thickness of the RV free wall was identified as a significant risk factor for CAD(P<0.05).According to the coronary angiography and interventional status,cases were further classified into an intervention group with balloon stent(n=88)and a non-intervention group(n=41).The anterior epicardial fat thickness of the RV free wall,pericardial fat thickness,and left AV groove epicardial fat thickness all demonstrated good diagnostic performance for CAD requiring intervention,with optimal cut-off values of 2.425 mm,3.415 mm,and 3.155 mm,respectively.The AS angle also showed diagnostic efficiency,with a cut-off value of 113.9°.The combination of the anterior epicardial fat thickness of the RV free wall and the AS angle yielded a higher diagnostic accuracy for CAD(AUC=0.776)than either parameter alone.When all four parameters were combined,the predictive performance further improved(AUC=0.789).Conclusion:The thickness of the anterior epicardial fat of the RV free wall,pericardial fat,and left atrioventricular groove epicardial fat measured by ultrasound can quantitatively reflect the pericardial fat.The anterior epicardial fat of the RV free wall is an independent risk for the occurrence of coronary heart disease.The combination of anterior epicardial fat of the RV free wall,pericardial fat,and left atrioventricular groove epicardial fat with the AS angle has a good predictive value for the degree of coronary artery stenosis.
