Effects of Short-Term Evolocumab Use on Vascular Endothelial Function in Patients with Ultra-High-Risk Atherosclerotic Cardiovascular Disease and Analysis of Its Influencing Factors

Abstract
Objective:To investigate the effects of short-term evolocumab use on vascular endothelial function in patients with ultra-high-risk atherosclerotic cardiovascular disease(ASCVD)and analyze its influencing factors.Methods:A total of 124 patients with ultra-high-risk ASCVD admitted to our hospital between March 2022 and January 2024 were retrospectively selected,and assigned to the atorvastatin group(42 patients,treated with atorvastatin),the evolocumab group(34 patients,treated with evolocumab),and the combination therapy group(48 patients,treated with atorvastatin plus evolocumab)based on treatment regimens.The vascular endothelial function parameters[flow-mediated dilation(FMD),tissue oxygen saturation(StO2),pulse wave velocity(PWV),and augmentation index(AIx)]were compared among the three groups.Based on clinical response to treatment,the patients were categorized into an effective treatment group and an ineffective treatment group.Cox regression analysis was employed to identify factors influencing vascular endothelial function.Results:After treatment,FMD and StO2 increased in all three groups,with both the combination therapy group and evolocumab group showing higher values than the atorvastatin group(P<0.05).PWV and AIx decreased in all three groups,with both the combination therapy group and evolocumab group showing lower values than the atorvastatin group(P<0.05).Multivariable Cox regression analysis revealed that the use of evolocumab may be a significant factor influencing vascular endothelial function.Conclusion:The short-term use of evolocumab can improve vascular endothelial function,reduce arterial stiffness,and optimize microcirculatory function in patients with ultra-high-risk ASCVD.The use of evolocumab may be associated with an improvement in vascular endothelial function.关键词
