Current status and strategies of building the primary care workforce in China

Abstract

Objective To conduct an in-depth research on the current status of the primary care work-force in China,to identify existing issues,and to provide optimization strategies.Methods A respondent-driven sampling(RDS)method was applied,where"seeds"(volunteer university students)were selected in various provinces and municipalities nationwide.These"seeds"distributed electronic or paper questionnaires to village/community doctors in their hometown,followed by targeted individual interviews based on given interview outlines.Results According to the data of 3 329 valid questionnaires collected,the national male-to-female ratio of vil-lage/community doctors was 2.13∶1;709 doctors were aged 55 and above,while 320 were between 18 and 34 years old;only 803 doctors had a bachelor's degree or higher;546 individuals did not pass the physician certifica-tion examination,and 53 did not register;1 252 persons did not meet the required training duration,and 1 452 did not meet the required training frequency,yet satisfaction with the current training situation was quite high;110 doctors and 296 doctors had not yet been covered by medical insurance and pension insurance;749 village/com-munity doctors had an average monthly disposable income below 2 000 yuan;Average scores on satisfaction with income(73.8 points/100 points),career prospects(77.0 points/100 points),and social security(pension in-surance:75.5 points/100 points,medical insurance:78.1 points/100 points)all remained relatively low.Con-clusions The national primary care workforce faces issues such as an unbalanced team structure,weak service capabilities,and low levels of support.Measures need to be taken to optimize the workforce structure,enhance training effectiveness,improve salary and benefits,and strengthen career sustainability,in order to promote the sustainable development of the primary care workforce.
