儿童难治性癫痫术后认知功能影响因素分析
Analysis of factors affecting cognitive function in children with refractory epilepsy after surgery

Abstract
Objective To investigate the independent predictive factors affecting cognitive outcomes in children with drug-resistant epilepsy following epileptogenic zone resection and to establish a multiple linear regression prediction model.Methods A total of 42 children with drug-resistant epilepsy who underwent epileptogenic zone resection at the Epilepsy Center of Capital Center for Children's Health,Capital Medical University between December 2016 and June 2022 were enrolled.A computerized cognitive task battery for epilepsy was used to assess preoperative and postoperative scores across 15 cognitive functions.Using the cognitive change value(%)of each cognitive function as the dependent variable,multiple linear regression models were employed to analyze the independent effects of preoperative scores,age at seizure onset,age at surgery,disease duration,and the number of preoperative anti-seizure medications(ASMs)on it.Results Among the 42 pediatric patients,30 were male(71.43%)and 12 were female(28.57%).The mean age at evaluation was(12.36±3.42)years,the mean age at seizure onset was(6.59±3.26)years,and the mean age at surgery was(9.71±2.70)years.Multiple linear regression analysis revealed that earlier age at seizure onset and a higher number of preoperative ASMs were independent risk factors for smaller degrees of improvement in multiple cognitive functions(all P<0.05).Age at surgery demonstrated an independent predictive effect in 15 cognitive functions,and age at onset in 9 functions(P<0.05).The number of preoperative ASMs was independently negatively correlated with postoperative scores in 6 cognitive functions,particularly executive functions(P<0.05).Conclusions Preoperative cognitive level,age at surgery,and age at seizure onset are important independent predictors of postoperative cognitive function,while preoperative polypharmacy is a potential negative factor.The multiple linear regression model constructed in this study has predictive value for postoperative cognitive outcomes and can provide a reference for individualized clinical prognosis assessment.关键词
