摘要

Abstract

Epilepsy is a chronic neurological disorder that imposes a substantial burden on public health and quality of life.Etiologically,it can be classified into structural,genetic,metabolic,infectious,and immune-related subtypes.Growing evidence highlights the pathogenic contribution of neuroinflammation to epileptogenesis.Anti-seizure medications(ASMs)remain the mainstay of therapy;however,approximately 30%of patients are pharmacoresistant to ASMs with diverse mechanisms of action,meaning they are unable to effectively control their epileptic seizures.Recent studies have identified exosomes as promising therapeutic vehicles capable of modulating inflammatory and immune responses.Exosomes may attenuate epileptic seizure burden by mediating immune reactions and mitigating neuroinflammation.This review summarizes current understanding of the neuroimmune mechanisms underlying epilepsy and delineates recent advances in exosome-mediated immunomodulation of neuroinflammation,thereby providing a theoretical framework and practical guidance for future research and clinical translation.
