慢性阻塞性肺疾病患者居家应用便携式肺功能仪的质量控制分析
Quality control analysis of the home-based application of portable spirometers in patients with chronic obstructive pulmonary disease

Abstract
Objective To describe the quality control results of home-based self-monitoring using portable spirometers in patients with chronic obstructive pulmonary disease(COPD),and to evaluate the feasibility of portable spirometers as a tool for long-term home-based self-monitoring.Methods A secondary analysis was conducted on data of 34 patients who uploaded remote spirometer data in the experimental group of the study"A New Model of Out-of-Hospital Management for COPD Patients:Effectiveness and Health Economics of Internet Hospitals and Remote Physiological Monitoring"from July 2022 to March 2024.Quality control results of home-based pulmonary function monitoring from patients were collected.The qualified rate of initial measurements and overall monitoring results,changes in the qualified rate over time,patient monitoring compliance,and influencing factors were analyzed.Results Among 48 patients in the experimental group,34(70.8%)conducted home monitoring and uploaded 2,163 pulmonary function results during the one-year follow-up period.According to the standards of the American Thoracic Society(ATS)and the European Respiratory Society(ERS),1,791 reports(82.8%)were qualified.The qualification rate of home monitoring showed slight fluctuations over time but remained generally stable.Compliance gradually decreased with the extension of time.Logistic regression analysis indicated that patient compliance was associated with gender,GOLD classification,inhaled medication,and time factors(P<0.05).Female patients and those with GOLD grade 3 had higher compliance,while patients using Long-cating muscarinic antagonist+Long-acting beta-agonists combination inhalation therapy had lower compliance.Compliance was higher in the first 9 months than in the last 3 months of the follow-up period.Conclusions The application of portable spirometers for long-term dynamic self-monitoring at home is feasible for patients with COPD.The compliance of long-term monitoring is associated with multiple factors.关键词
