Abstract
Objective To observe the effect of preoperative use of stellate ganglion block(SGB)on postoperative arteriovenous fistula(AVF)maturation in patients undergoing arteriovenous fistuloplasty.Methods A total of 88 patients undergoing cephalic vein-radial artery autologous fistuloplasty under elective axillary brachial plexus block anesthesia at Nanjing First Hospital from August 2022 to December 2023 were selected,and all patients were randomly divided by random number table method into ultrasound-guided preoperative injection of SGB(SGB group,n=44)or ultrasound scan only(control group,n=44).Ultrasound was used to measure the diameter of the cephalic vein and radial artery and the fistula blood flow of the two groups at 3 hours and 2 months postoperatively.The fistula maturation rate and the number of days required for the maturation of patients with fistula maturation were recorded.The postoperative complications of AVF and the incidence of complications related to postoperative nerve block were recorded.Results Compared with the control group,the postoperative fistula maturation rate and the number of fistula maturation days were significantly shortened in the SGB group(P<0.05).Compared with the control group,the diameter of the cephalic vein and radial artery in the SGB group was significantly widened and the fistula blood flow was significantly increased at 3 hours and 2 months postoperatively(P<0.05).Compared with the control group,the incidence of fistula stenosis and thrombosis after AVF was significantly reduced in the SGB group(P<0.05).There was no significant difference in other postoperative complications between the two groups(P>0.05);Two patients in the SGB group developed hoarseness after surgery,and no other nerve block-related complications were observed.Conclusions Preoperative ultrasound-guided SGB can dilate blood vessels,reduce the occurrence of stenosis and thrombosis,and have a positive effect on the maturation of arteriovenous fistulas.关键词
