不同手术方案治疗胆囊结石合并胆总管结石的疗效分析
Therapeutic effects analysis of different surgical regimens in the treatment of cholecystolithiasis and choledocholithiasis

Abstract
Objective:To compare the clinical outcomes of different surgical strategies for patients with cholecystolithiasis and choledocholithiasis.Methods:From Jan.2022 to May 2024,120 patients with concomitant cholecystolithiasis and choledocholithiasis were randomly assigned by a simple randomization method to two groups.Group A(n=60)underwent laparoscopic cholecystectomy(LC)combined with laparoscopic common bile duct exploration and primary suture.Group B(n=60)underwent LC combined with endoscopic sphincterotomy(EST)with small incision plus balloon dilation.Perioperative outcomes,liver function indices,pain scores,complication rates,and calculi clearance rates were compared between the two groups.Results:Group B demonstrated significantly shor-ter operative time,time to first flatus,and hospital stay,as well as less intraoperative blood loss and drainage volume compared to Group A(P<0.05).Postoperative levels of aspartate aminotransferase,alanine aminotransferase,total bilirubin,and pain visual analog scale cores decreased in both groups,with Group B showing lower values than Group A(P<0.05).The complication rate was significantly lower in Group B(5.00%vs.20.00%,P<0.05).There was no significant difference in the calculi clearance rate between Group B and Group A(100.00%vs.96.67%,P>0.05).Conclusions:Both surgical strategies effectively clear calculi.However,LC combined with EST with small incision plus balloon dilation provides superior outcomes in terms of perioperative indicators,liver function preser-vation,pain reduction,and lower complications.关键词
