腹腔镜困难胆囊切除术中吲哚菁绿荧光显像技术的应用效果
Application effect of indocyanine green fluorescence imaging technology in laparoscopic difficult cholecystectomy

Abstract
Objective:To investigate the application value of indocyanine green(ICG)fluorescence imaging in laparoscopic difficult cholecystectomy.Methods:Eighty-six patients who met the criteria for difficult cholecystectomy between Feb.2021 and May 2024 were enrolled and randomly assigned using the random drawing of lots to either the ICG group(n=43)or the conventional group(n=43).The conventional group underwent standard laparoscopic cholecystectomy,while the ICG group underwent surgery guided by ICG fluorescence navigation.The bile duct identification time,total operative time,intraoperative blood loss,postoperative drainage dura-tion,hospital stay,inflammatory marker levels,and incidence of perioperative complications were compared between the two groups.Results:There were no conversions to open surgery in either group.One patient in the conventional group underwent gallbladder drain-age.Compared to the conventional group,the ICG group demonstrated significantly shorter bile duct identification time[(0.80±0.70)min vs.(2.55±2.26)min],total operative time[(63.02±23.88)min vs.(82.26±34.05)min],postoperative drainage duration[(2.14±1.34)d vs.(2.91±1.63)d],and hospital stay[(5.70±2.11)d vs.(7.05±2.78)d].The ICG group also had significantly less intr-aoperative blood loss[(13.37±8.29)mL vs.(17.21±8.11)mL],lower postoperative levels of inflammatory markers,and a lower in-cidence of perioperative complications(P<0.05).Conclusions:The application of ICG fluorescence imaging in laparoscopic difficult cholecystectomy is safe and feasible.It facilitates quicker identification of the biliary anatomy,accelerates the operative process,reduces intraoperative blood loss,shortens postoperative recovery time,and lowers postoperative inflammatory response and complication rates.关键词
