Establishment and validation of a predictive model for postoperative cough hypersensitivity syndrome in lung cancer patients after single-port thoracoscopic surgery via intercostal approach

Abstract

Objective:To establish and validate a prediction model for postoperative cough hypersensitivity syndrome(PCHS)in lung cancer patients undergoing single-port thoracoscopic surgery via the intercostal approach.Methods:A total of 224 lung cancer patients who underwent single-port thoracoscopic surgery via the intercostal approach from Feb.2021 to Dec.2024 were included.Patients were divided into PCHS and non-PCHS groups based on the occurrence of PCHS.Clinical data of the two groups,including gender,age,tumor pathological features,and perioperative indicators,were collected,compared,and analyzed.Multivariate logistic regression analysis was used to identify risk factors for PCHS,and a nomogram prediction model was constructed based on the multiva-riate analysis results.The model was evaluated using receiver operating characteristic curve.Results:PCHS occurred in 91 cases(40.63%).In the PCHS group,the proportions of female patients,right-sided surgery,non-smoking history,comorbidity of chronic obstructive pulmonary disease(COPD),peribronchial lymph node dissection,upper lobe resection,prolonged anesthesia time,and the postoperative white blood cell count were higher,with statistically significant differences(P<0.05).Logistic regression analysis showed that non-smoking history,history of COPD,peribronchial lymph node dissection,upper lobe resection,increased anesthesia time,and elevated postoperative white blood cell count were independent risk factors for PCHS(P<0.05).Receiver operating characteristic curve analysis indicated that the nomogram model established based on the multivariate analysis results had a sensitivity of 80.2%and a spe-cificity of 68.4%.Conclusions:The nomogram prediction model constructed based on independent risk factors such as non-smoking history and COPD history has good discriminability and calibration.It can provide an intuitive and practical quantitative tool for early identification of high-risk patients for PCHS after single-port thoracoscopic lung cancer surgery,which is helpful for implementing targe-ted intervention measures.
