单孔胸腔镜肺段切除术对非小细胞肺癌患者围手术期指标及肺功能的影响
Effect of single-port thoracoscopic segmentectomy on perioperative outcomes and pulmonary function in patients with non-small cell lung cancer

Abstract
Objective:To investigate the impact of single-port thoracoscopic segmentectomy on pulmonary function and progno-sis in patients with non-small cell lung cancer(NSCLC).Methods:A total of 400 NSCLC patients admitted between Oct.2019 and Nov.2022 were enrolled and divided into two groups according to the surgical approach:the single-port group(n=200,undergoing sin-gle-port thoracoscopic segmentectomy),and the three-port group(n=200,undergoing three-port thoracoscopic segmentectomy).Peri-operative parameters,pulmonary function(including maximum voluntary ventilation,forced vital capacity,and forced expiratory volume in one second),and immune markers(CD4+,CD8+,CD4+/CD8+ratio,CD16+CD56+)were compared between the two groups.Results:Compared with the three-port group,the single-port group showed significantly shorter incision length and postoperative drain-age duration,less intraoperative blood loss,but longer operative time(P<0.05).Postoperatively,the levels of forced vital capacity and forced expiratory volume in one second in the single-port group were higher than both the preoperative levels and those in the three-port group.Although maximum voluntary ventilation decreased in both groups after surgery,it was higher in the single-port group than in the three-port group(P<0.05).Regarding immune function,the single-port group had higher levels of CD4+,CD4+/CD8+ratio,and CD16+CD56+,and a lower level of CD8+than the three-port group(P<0.05).Conclusions:Single-port thoracoscopic segmentectomy for NSCLC can effectively improve certain perioperative outcomes,better preserve pulmonary function,and mitigate the impact on immune function,which suggests it is worthy of widespread clinical adoption.关键词
