高危孕产妇孕早期肥胖与不良妊娠结局的关联
Association between first-trimester obesity and adverse pregnancy outcomes in high-risk pregnant women

Abstract
Objective To explore the association between first-trimester obesity and adverse pregnancy outcomes in high-risk pregnant women,and to provide evidence for weight management in women of childbearing age and during early pregnancy.Methods A total of 4,878 high-risk pregnant women registered in Quzhou from January 1,2023,to September 30,2024,were included.Participants were grouped based on the presence of first-trimester obesity as the exposure factor.Differences in adverse delivery outcomes between the two groups were compared,and binary logistic regression models were used for multivariate analysis.Results Among the 4,878 high-risk pregnant women,316(6.5%)were classified as obese and 4,562(93.5%)as non-obese.Univariate analysis showed that obesity,advanced maternal age,number of pregnancies,history of adverse pregnancy,and the presence of other high-risk factors were associated with preterm birth and cesarean section(all P<0.05).Multivariate regression an`alysis indicated that obesity(OR=1.738,95%CI:1.187-2.544,P=0.004)and advanced maternal age(OR=1.668,95%CI:1.272-2.186,P<0.001)were risk factors for preterm birth,and obesity(OR=2.197,95%CI:1.714-2.815,P<0.001)and advanced maternal age(OR=1.659,95%CI:1.416-1.943,P<0.001)were risk factors for cesarean section.Conclusions First-trimester obesity is associated with adverse pregnancy outcomes in high-risk pregnant women and constitutes an independent risk factor.Weight management in early pregnancy should be emphasized to reduce related health risks in this population.关键词
