Abstract
Objective:To investigate how sleep quality relates to glycemic control and perinatal outcomes in pregnant women with diabetes,and to assess glycemic changes after a sleep-focused intervention in those with poor sleep.Methods:Eighty-six pregnant women with diabetes attending our hospital's outpatient department from July 2022 to July 2024 were enrolled.Based on the Pitts-burgh Sleep Quality Index(PSQI),participants were categorized into a low sleep quality group(PSQI≥7;n=43)and a high sleep quality group(PSQI<7;n=43).Between-group comparisons included anxiety and depression(scores and prevalence),in-sulin use,glycemic indices[fasting plasma glucose(FPG),1 hour postprandial glucose(1 h PG),2 hour postprandial glucose(2 h PG),glycosylated hemoglobin(HbA1c),lipid profile[low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),total cholesterol(TC),triglycerides(TG)],blood pressure,and pregnancy outcomes(obstetric complica-tions,cesarean delivery,neonatal complications).In the low sleep quality group,glycemic indices were reassessed after a sleep-fo-cused intervention.Results:Compared with the low sleep quality group,the high sleep quality group had lower PSQI scores,anxie-ty and depression scores and prevalence,insulin use,FPG,1 h PG,2 h PG,HbA1c,LDL-C,TC,TG,systolic blood pressure,ob-stetric complications,cesarean delivery rate,and neonatal complications(all P<0.05).Within the low sleep quality group,FPG,1 h PG,2 h PG,and HbA1c were significantly reduced after the sleep intervention compared with baseline(all P<0.05).Conclu-sion:Better sleep quality in pregnant women with diabetes is associated with improved glycemic control,a more favorable cardio-metabolic profile,and better pregnancy outcomes.Improving sleep among those with poor sleep may help reduce blood glucose levels.关键词
