非糖尿病稳定性冠心病患者血清淀粉样蛋白A/白蛋白比值与冠状动脉血流储备分数的相关性及其临床价值
Correlation between serum amyloid A/albumin ratio and fractional flow reserve in non-diabetic patients with stable coronary heart disease and its clinical implications

Abstract
Objective To explore the correlation between serum amyloid A/albumin ratio(SAR)and fractional flow reserve(FFR)in non-diabetic patients with stable coronary heart disease,and assess the clinical value of SAR in predicting coronary ischemia.Methods Clinical data from 98 non-diabetic patients with stable coronary heart disease from January 2022 to July 2024,whose coronary lesions(50%～90%stenosis)were confirmed by coronary angiography at the Lishui Central Hospital,were collected and analyzed.FFR was measured during coronary angiography.Based on FFR results,patients were categorized into an FFR≥0.8 group(55 cases)and an FFR<0.8 group(43 cases).Patients'data were compared between the two groups.Pearson correlation analysis was employed to evaluate the correlation between SAR and FFR.Multivariate Logistic regression analysis was used to indentify factors influencing coronary ischemia.Receiver operating characteristic(ROC)curve was conducted to assess the predictive value of SAR for coronary ischemia.Results Compared to the FFR≥0.8 group,patients in the FFR<0.8 group exhibited significantly higher values of white blood cell count,hypersensitive C-reactive protein(hs-CRP),serum amyloid A(SAA),and SAR,while albumin(ALB)was significantly lower(all P<0.05).Pearson correlation analysis revealed a negative correlation between SAR and FFR(r=-0.637,P<0.01).Multivariate Logistic regression analysis indicated that SAR was an influencing factor for coronary ischemia(OR=0.403,P<0.05).ROC curve analysis demonstrated that the predictive efficacy of SAR for coronary ischemia was superior to that of hs-CRP and white blood cell count.The optimal diagnostic cut-off value for SAR was 0.18,with a Youden index of 0.573,sensitivity of 79.10%,specificity of 78.20%,and an area under the curve of 0.872.Conclusion SAR may serve as a predictive biomarker for coronary ischemia in non-diabetic patients with stable coronary heart disease,and provide reference value for guiding revascularization in this population.关键词
