Abstract
Objective To analyze the value of left ventricular high voltage in assessing left ventricular hypertrophy(LVH)and cardiovascular event risk in elderly hypertensive patients.Methods A total of 360 patients with hypertension admitted to Anqing Municipal Hospital from January 2020 to April 2021 were prospectively enrolled and divided into an LVH group(115 cases)and a non-LVH group(245 cases)according to the occurrence of LVH.General clinical data,24-hour ambulatory blood pressure parameters,and the prevalence of left ventricular high voltage of the two groups were analyzed and compared.Multivariate Logistic regression analysis was used to identify influencing factors for LVH.Elderly hypertensive patients with LVH were followed up(until the occurrence of a cardiovascular event or the cutoff date of April 1,2024)to observe the occurrence of cardiovascular events.These patients were then categorized into groups with and without cardiovascular events,and the prevalence of left ventricular high voltage between the two groups was compared.A ROC curve was plotted to analyze the predictive value of left ventricular high voltage for the risk of cardiovascular events in elderly hypertensive patients with LVH.Results Among the 360 elderly hypertensive patients,115(31.94%)were diagnosed with LVH.There were significant differences in hypertension grading,total cholesterol(TC),and homocysteine(Hcy)between the two groups(t/χ2=34.093,4.875,8.925;all P<0.05).The 24-hour pulse pressure standard deviation(24 h PP-SD)also differed significantly between the two groups(t=3.664,P<0.05).The left ventricular high voltage rate in the LVH group was significantly higher than that in the non-LVH group(χ2=36.763,P<0.05).Multivariate Logistic regression analysis identified hypertension grading(OR=2.581,95%CI=2.150-3.099),TC(OR=2.146,95%CI=1.849-2.491),Hcy(OR=1.468,95%CI=1.392-1.548),24 h PP-SD(OR=1.277,95%CI=1.216-1.340),and left ventricular high voltage(OR=3.606,95%CI=2.689-4.836)as risk factors for LVH(P<0.05).A total of 110 hypertension patients complicated with LVH completed the follow-up,of whom 21 cases(19.09%)experienced cardiovascular events.The incidence of left ventricular high voltage was significantly higher in patients with cardiovascular events(90.48%)compared to those without(68.54%)(χ2=4.122,P<0.05).ROC curve analysis showed that left ventricular high voltage had predictive value for cardiovascular events in these patients,with an AUC of 0.610(95%CI=0.512-0.701).Conclusion Left ventricular high voltage is a risk factor for LVH in elderly hypertensive patients and has predictive value for cardiovascular events in elderly hypertensive patients with LVH.关键词
