维持性血液透析患者透析前血清镁离子水平与透析中低血压的相关分析
Association between pre-dialysis level of serum magnesium ion and intradialytic hypotension in maintenance hemodialysis patient

Abstract
Objective To investigate the association of serum Mg2+level before dialysis with intradialytic hypotension(IDH)in maintenance hemodialysis(MHD)patients.Methods A cross-sectional analysis was conducted on 202 patients who underwent MHD in the outpatient department of Ningxia Medical University General Hospital from September to December 2022.The MHD patients were divided into IDH group(n=56)and NIDH group(n=146)based on the presence or absence of IDH.Spearman correlation analysis was used to evaluate the relationship between pre-dialysis level of serum Mg2+and blood pressure.Multivariate logistic regression was employed to explore influencing factors for IDH.ROC curve was used to analyze the predictive value of pre-dialysis serum Mg2+for IDH.Results In the 202 MHD patients,56 patients(27.7％)suffered from IDH.Compared with those in NIDH group,patients with IDH presented higher pre-dialysis serum Mg2+levels(1.28 mmol/L vs.1.14 mmol/L,P＜0.05).Correlation analysis showed that pre-dialysis serum Mg2+level was negatively correlated with systolic blood pressure at 3 hours(r=-0.202,P=0.004)and 4 hours after dialysis(r=-0.197,P=0.005),and was positively correlated with the change in intradialytic systolic blood pressure(r=0.204,P=0.004)and the change in intradialytic mean arterial pressure(r=0.205,P=0.003).After adjustment for confounding factors,multivariate logistic regression showed that pre-dialysis serum Mg2+level greater than 1.15 mmol/L was an independent risk factor for IDH.Conclusion Higher pre-dialysis serum Mg2+level may be one of the independent risk factors for IDH in MHD patients.Pre-dialysis serum Mg2+levels greater than 1.15 mmol/L is associated with a higher incidence of IDH.关键词
