An Integrated Framework of Cochlear Implantation Guided by Auditory Evoked Potentials:Evaluation,Monitoring,and Optimization

Abstract
Cochlear implantation has become the most effective intervention for severe to profound sensorineural hearing loss.Auditory evoked potentials(AEPs),as objective and quantitative electrophysiological biomarkers,play an essential role throughout the entire cochlear implantation process—from preoperative evaluation and intraoperative monitoring to postoperative outcome prediction.This review summarizes the latest progress of major AEPs techniques,including electrical evoked auditory brainstem response(EABR),electrical evoked compound action potential(ECAP),electrocochleography(ECochG),electrical evoked middle latency response(EMLR),and cortical auditory evoked potential(CAEP).Discussing evidence from both clinical and animal studies,we highlight their distinct roles in auditory nerve integrity assessment,intraoperative safety control,neural plasticity tracking,and rehabilitation prognosis.AEPs have evolved from diagnostic tools into a core neurophysiological platform spanning preoperative assessment for cochlear implantation through postoperative parameter tuning and long-term management,providing a robust biological foundation for precision auditory rehabilitation and intelligent programming.关键词
