中医经方分期论治儿童特应性皮炎的临床实践与思考
Clinical practice and theoretical considerations of stage-specific treatment with classical Chinese medical formulas in pediatric atopic dermatitis

Abstract
Pediatric atopic dermatitis(AD)is characterized by recurrent pruritus,erythema,exudation,and lichenification.Traditional Chinese medicine(TCM)classifies it within the categories of"Four Flexures Wind"or"Infantile Eczema".Based on the holistic perspective and the principle of syndrome differentiation and treatment in TCM,combined with the clinical staging characteristics of AD,this study explores the flexible application of classical formulas in the stage-specific management of pediatric AD.Through retrospective case analysis,we propose the concept of"treating beyond the skin lesions",which emphasizes starting from systemic pathogenesis analysis,governing the exterior to infer the interior,and harmonizing the qi and blood of the organs to eliminate pathogens and reinforce healthy qi.Building upon the theoretical framework of classical TCM formulas,this work systematically interprets the core pathogenesis of"deficiency in origin and excess in manifestation"in pediatric AD and its syndrome evolution patterns:"During acute stages,pathogens reside superficially and require dispersion through venting,in chronic stages,prolonged illness penetrates collaterals,depleting fluids and blood,while all qi stagnation disorders pertain to the lung domain".By analyzing stage-specific skin lesion characteristics,constitutional predispositions,and concurrent symptoms through syndrome differentiation,we establish a"temporally phased regulation strategy using classical formulas".This strategy centers on canonical prescriptions from Shanghan Lun such as Ephedra-Cinnamon Combination,Maxingyigan Decoction,and Peach Kernel-Carthamus Four Substances Decoction.A sequential treatment protocol is developed:"Expelling pathogens and resolving turbidity in acute phases.consolidating root qi during remission,with pulmonary regulation as the therapeutic pivot in stage-specific management".This approach enhances clinical efficacy,stabilizes disease progression,reduces the dosage/duration of Western medications,and minimizes adverse effects,thereby providing theoretical and clinical frameworks for modernizing classical formula applications in pediatric AD.It underscores the unique value of integrating Chinese and Western medicine in managing refractory dermatological disorders.关键词
