生活经济压力对中老年乳腺癌患者抑郁的链式中介模型研究
The chain mediation model of financial stress on depression through fear of progression and sleep quality in middle-aged and elderly breast cancer patients

Abstract
Objective To explore the effect of financial stress on depression among middle-aged and elderly breast cancer patients and to analyze the chained mediating role of fear of progression(FoP)and sleep quality.Methods A cross-sectional study was conducted among 262 middle-aged and elderly female breast cancer patients,recruited from Nanfang Hospital Southern Medical University,and Guangdong Provincial People's Hospital,Guangdong Academy of Medical Sciences,Southern Medical Universityl from May 2018 to March 2021.Financial stress,depression,FoP,and sleep quality were assessed using a demographic questionnaire,the patient health questionnaire(PHQ-9),the fear of progression questionnaire-short form(FoP-Q-SF),and the Athens insomnia scale(AIS).Data were analyzed using SPSS 29.0 and Process 4.2 to construct a chain mediating model and perform a Bootstrap test.Results Patients experiencing moderate-to-high financial stress showed a significantly higher prevalence of depressive symptoms than those with no-to-low financial stress(62.5%vs.38.5%)the difference is statistically significant(P<0.01).FoP and insomnia severity were positively correlated with depressive symptoms(r=0.550 and 0.579,all P<0.01).The chained mediation model demonstrated that financial stress indirectly influenced depressive symptoms through FoP and sleep quality,accounting for 68.15%of the total effect.The pathway"financial stress→ FoP→ sleep quality→ depression"showed significant indirect effects(effect=0.469,95%CI=0.217-0.788).Conclusion Financial stress did not directly predict depression of life in middle-aged and elderly breast cancer patients but exerted indirect effects through fear of progression and sleep quality.These findings highlight the combined influence of financial stress,fear of progression,and sleep quality on the mental health of middle-aged and elderly breast cancer patients.关键词
