腰神经根阻滞联合骶丛阻滞对髋关节置换术患者镇痛效果和术后恢复的影响
Effect of lumbar nerve root block combined with sacral plexus block on analgesic effect and postoperative recovery in patients undergoing hip replacement

Abstract
Objective To investigate the effects of lumbar nerve root block combined with sacral plexus block plus general anesthesia on perioperative vital signs,postoperative analgesia and postoperative recovery in patients undergoing hip replacement.Methods From September 2024 to May 2025,60 patients undergoing primary unilateral hip replacement in Zhangzhou Municipal Hospital of Fujian Province were selected.The patients were divided into control group(simple general anesthesia,n=30)and observation group(lumbar nerve root block combined with lumbosacral plexus block plus general anesthesia,n=30)by random number table.The heart rate(HR)and mean arterial pressure(MAP)were recorded before anesthesia(T0),at intubation(T1),at prosthesis implantation(T2)and at the end of operation(T3).The visual analogue scale(VAS)score at 2,6,12,24,48 and 72 h after operation,postoperative bed rest time,extubation time and anesthesia recovery time were recorded.The 40-item quality of recovery score(QoR-40)was used to evaluate the recovery quality at 24,48 and 72 h after operation,and the adverse reactions were counted.Results At time points T1 to T3,the observation group demonstrated a lower heart rate and a higher mean arterial pressure compared to the control group,with statistically significant differences(P<0.05).Furthermore,from 2 to 72 hours post-surgery,the observation group exhibited consistently lower VAS scores,the postoperative extubation time and recovery time were earlier,and had a shorter bed-rest duration.They also achieved higher scores on the QoR-40 scale at 24 to 72 hours postoperatively,alongside a lower total incidence of adverse effects.All these comparisons showed statistical significance(P<0.05).Conclusion lumbar nerve root block combined with sacral plexus block plus general anesthesia can effectively stabilize the perioperative vital signs of patients undergoing hip replacement,enhance the analgesic effect,promote early postoperative rehabilitation,improve the quality of postoperative recovery,and reduce adverse reactions.关键词
