毫火针治疗白癜风有效性和安全性的临床观察:单中心回顾性观察研究
Clinical Observation on the Efficacy and Safety of Haohuo Needling in the Treatment of Vitiligo:A Single-Center Retrospective Observational Study

Abstract
Objective In order to evaluate the clinical efficacy and safety of the fire-needle comprehensive therapy for vitiligo and to provide evidence for optimizing integrated traditional Chinese and Western medicine treatment strategies.Methods A retrospective cohort study was conducted,including 152 vitiligo patients who attended the outpatient and inpatient departments of the dermatology department,Xinjiang Uygur autonomous region hospital of traditional Chinese medicine,between January 2021 and December 2024.Based on whether patients received fire-needle therapy at the initiation of systemic treatment,two naturally formed cohorts were identified:A fire-needle exposure group and a non-exposure group receiving conventional therapy alone.Changes in the vitiligo area scoring index(VASI)before and after treatment,overall clinical effectiveness,and the incidence of adverse reactions were compared between the two groups to evaluate the efficacy and safety of fire-needle therapy.Results The overall clinical effective rate in the exposure group was significantly higher than that in the non-exposure group[92.11％(70/76)vs.77.63％(59/76),x2=7.767,P=0.033].After treatment,the exposure group showed a greater reduction in total VASI scores compared with the non-exposure group(Z=-2.39,P=0.017).Significant improvements were also observed in trunk(Z=-2.03,P=0.042),extremities(Z=-2.10,P=0.036)and acral(Z=-2.02,P=0.043)regions(all P＜0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P＞0.05).Conclusion The fire-needle comprehensive therapy demonstrates superior clinical efficacy compared with conventional pharmacological treatment alone,particularly in promoting repigmentation of the trunk,extremities and acral regions,with a favorable safety profile.It may serve as an effective adjunctive therapy for vitiligo in clinical practice.关键词
