肌骨超声引导下张力平衡针法对脑卒中恢复期手痉挛的临床研究
Clinical Study of Musculoskeletal Ultrasound-Guided Tension-Balancing Acupuncture for Hand Spasm in Stroke Patients During Recovery Phase

Abstract
Objective:To evaluate the clinical efficacy of the tension-balancing acupuncture method under musculoskeletal ultrasound guidance for hand spasm in stroke patients during the recovery stage.Methods:A total of 117 patients with post-stroke hand spasm in the recovery phase were randomly assigned to the tension-balancing acupuncture group(n=59)and the conventional acupuncture group(n=58).Both groups received standard therapy consisting of scalp acupuncture combined with rehabilitation training;on which basis,the tension-balancing acupuncture group received musculoskeletal ultrasound-guided tension-balancing acupuncture,whereas the conventional group received routine acupuncture.All patients underwent continuous treatment for 4 weeks.Clinical efficacy was evaluated,and outcome measures included the Modified Ashworth Scale(MAS)for spasticity and the hand function subscale of the simplified Fugl-Meyer Motor Assessment(FMA)before and after the treatment.The Modified Barthel Index(MBI)was used to assess activities of daily living.Adverse reactions were also recorded and compared between the two groups.Results:The total effective rate was 79.66％(47/59)in the tension-balancing acupuncture group,which was significantly higher than 56.89％(33/58)in the conventional acupuncture group(P＜0.05).The hand spasm scores were decreased in both groups,with lower scores observed in the tension-balancing group compared with the conventional group(P＜0.05).Fugl-Meyer scores were increased in both groups,with higher scores in the tension-balancing acupuncture group(P＜0.05).Modified Barthel Index(MBI)scores were improved in both groups,with greater improvement in the tension-balancing acupuncture group(P＜0.05).No adverse reactions occurred in either group during the treatment period.Conclusion:Musculoskeletal ultrasound-guided tension-balancing acupuncture demonstrates good clinical efficacy in treating hand spams in stroke patients during the recovery phase.It can effectively reduce muscle spasticity to a certain extent and shows acceptable safety.关键词
