从"通腑泄浊补虚"探讨脓毒症肠源性感染的病机与证治
A Study on the Pathogenesis,Syndrome Differentiation and Treatment of Intestinal-De-rived Infection in Sepsis with the Method of"Unblocking the Fu Organs,Discharging Turbidity and Tonifying Deficiency"

Abstract
Intestinal-derived infection in sepsis is characterized by core pathological manifestations such as intestinal barrier injury and flora imbalance.The therapy of"Unblocking the Fu-viscera,Expelling Turbid Tox-in,and Tonifying Deficiency"is guided by the theories that"the Fu-viscera function well when unobstructed"and"treating intermingled deficiency and excess with simultaneous application of purgation and tonification".Targeting the pathogenesis of"obstructed Fu-viscera Qi,internal accumulation of turbid toxin,and vital Qi de-ficiency with toxin invasion",it restores the ascending and descending movement of Qi by unblocking Fu-vis-cera Qi,expelling turbid toxin,and strengthening vital Qi to eliminate pathogenic factors.This study clarifies that the core pathogenesis of intestinal-derived infection in sepsis lies in blocked fu-viscera Qi,intermingling of turbid toxin,and Qi-Blood deficiency,which lead to blazing internal toxic heat,intestinal obstruction,and vi-tal Qi deficiency complicated by toxin invasion.It proposes that the therapeutic method of"Unblocking the Fu-viscera,Expelling Turbid Toxin,and Tonifying Deficiency"should follow the principle of"taking unobstruction as smooth flow and applying purgation and tonification simultaneously".Clinical treatment should be based on syndrome differentiation in three stages:fu-viscera excess and obstruction,internal attack of turbid toxin,and vital Qi deficiency with toxin invasion.The core prescription is modified on the basis of Dachengqi Decoction ac-cording to specific syndromes.The therapy of"Unblocking the Fu-viscera,Expelling Turbid Toxin,and Tonify-ing Deficiency"embodies the characteristics of holistic regulation and treatment based on syndrome differentia-tion in traditional Chinese medicine,and can provide a practical approach for the treatment of intestinal-derived infection in sepsis.关键词
