基于态靶辨治理论探讨IgA肾病辨治策略
Discussion on the strategies for the syndrome differentiation and treatment of IgA nephropathy based on the theory of state-target differentiation and treatment

Abstract
IgA nephropathy(IgAN)is one of the most common pathological types of primary glomerular diseases,and the immune response induced by intestinal or respiratory mucosal infection is its main pathogenesis.Its prognosis varies greatly in clinical practice,with only a few achieving remission.Traditional Chinese medicine exhibits significant advantages in improving related indexes,maintaining disease stability and protecting renal function.Under the guidance of Professor Tong Xiaolin's theory of state-target differentiation and treatment,it is maintained that the main characteristics of IgAN are deficiency in its root cause and excess in its manifestations,insufficient body endowment and invasion of external pathogens,while external pathogens invading and lurking in the kidney collaterals,internal damage to vital qi,endogenous toxins,and viscera fatigue contribute to the main pathogeneses of this disease.According to its evolution characteristics,IgAN can be classified into three states of traditional Chinese medicine:bi,pathogen accumulation,and body decaying,which correspond to its"acute stage,compensatory stage and fatigue stage"respectively,denoting the development and evolution process of the disease.The core prescriptions for these three states are Shengjiang San,Huaban Decoction and Tongshi Qidan Junzhi Decoction.On the basis of the above,it is proposed that treatment be tailored to individual syndromes,and targeted formulas and drugs be refined to construct a theoretical system for the treatment of IgA nephropathy based on state-target differentiation,aiming to provide a new syndrome differentiation and treatment model for clinical TCM treatment of the disease.关键词
