视神经脊髓炎谱系疾病患者Th与Treg细胞水平变化及临床意义
Clinical significance and changes of Th and Treg cell levels in neuromyelitis spectrum diseases

Abstract
Objective To investigate the changes of helper T cells(Th)and regulatory T cells(Treg)in patients with neuromyelitis optica spectrum disorder(NMOSD)and their clinical significance.Methods Ninety-two patients with NMOSD admitted to Xianyang Hospital,Yan'an University from January 2020 to January 2025 were selected as NMOSD group.According to the expanded disability status scale(EDSS),the patients were divided into severe group(EDSS≥5 points)with 44 cases and mild group(EDSS<5 points)with 48 cases.In addition,50 healthy subjects who underwent physical examination during the same period were selected as control group,and the levels of Th and Treg cells were detected by flow cytometry.The clinical data of NMOSD group and control group,severe group and mild group were compared.Pearson correlation analysis was used to analyze the correlations among Th1/Th2,Th17/Treg and EDSS scores.The main influencing factors for NMOSD were analyzed by Logistic regression.ROC curve was used to analyze the efficacy of Th1/Th2 and Th17/Treg in predicting severe NMOSD.Results The expression levels of Th1 cell,Th17 cell,Th1/Th2 and Th17/Treg in the NMOSD group were higher than those in the control group,while the lymphocyte count and Treg cell expression level were lower than those in the control group(P<0.05).The proportion of patients with spinal cord lesion,WBC count,neutrophil count,Th1 cell expression,Th17 cell expression,Th1/Th2 and Th17/Treg in the severe group were higher than those in the mild group,and the lymphocyte count and Treg cell expression were lower than those in the mild group(P<0.05).Correlation analysis showed that Th1/Th2,Th17/Treg were positively correlated with EDSS score(r=0.577,0.693,P<0.001).Logistic regression analysis showed that spinal cord involved segments(OR=1.867,95%CI:1.392-5.281),Th1/Th2(OR=1.582,95%CI:1.283-5.172),Th17/Treg(OR=1.446,95%CI:1.193-4.923)were the main influencing factors for NMOSD severity(P<0.05).ROC curve analysis showed that the AUC values of Th1/Th2,Th17/Treg and their combined prediction of severe NMOSD were 0.712(95%CI:0.605-0.820),0.733(95%CI:0.629-0.836)and 0.865(95%CI:0.789-0.940),respectively,P<0.05.Conclusion Th1/Th2 and Th17/Treg increase significantly in the NMOSD patients and are correlated with the severity of the disease,which could be used as auxiliary indicators to evaluate NMOSD.关键词
