外周血MHR RPR与急性脑梗死取栓患者短期预后的相关性
Correlations between peripheral blood MHR,RPR and short-term prognosis in patients with acute cerebral infarction undergoing thrombectomy

Abstract
Objective To investigate the correlation and clinical significance of monocyte-to-high-density lipoprotein cholesterol ratio(MHR)and red cell distribution width-to-platelet ratio(RPR)with short-term prognosis in patients with acute cerebral infarction undergoing thrombectomy.Methods A retrospective analysis was conducted on the clinical data of 87 patients with acute cerebral infarction who underwent thrombectomy in the First Affiliated Hospital of Bengbu Medical University from January 2021 to October 2024.Based on the 3-month modified Rankin scale(mRS)score,patients were divided into good prognosis group(mRS≤2)and poor prognosis group(mRS>2).General clinical data,MHR,and RPR levels were compared between the two groups.Spearman correlation analysis was used to assess the relationship between these biomarkers and mRS scores.Logistic regression analysis was performed to identify factors influencing prognosis.ROC curves were plotted to evaluate the predictive performance of MHR and RPR for prognosis in acute ischemic stroke patients undergoing thrombectomy.Results The poor prognosis group exhibited significantly higher levels of RDW,RPR,MHR,monocyte count,admission NIHSS score,onset-to-recanalization time,and proportion of patients with hemorrhagic transformation compared to the good prognosis group,while HDL-C levels were significantly lower(P<0.05).Multivariate Logistic regression analysis revealed that RPR(OR=1.236,95%CI:1.052-1.451),MHR(OR=1.036,95%CI:1.014-1.059),and admission NIHSS score(OR=1.238,95%CI:1.031-1.486)were all statistically significant in the model(P<0.05)and served as independent risk factors for poor prognosis.Spearman correlation analysis demonstrated significant positive correlations between both MHR(r=0.469,P<0.001)and RPR(r=0.499,P<0.001)with mRS scores.ROC curve analysis showed that the areas under the curve(AUC)for predicting poor prognosis were 0.740 for RPR,0.778 for MHR,and 0.841 for their combination,P<0.05.Conclusion Elevated MHR and RPR levels are associated with short-term prognosis in acute ischemic stroke patients undergoing thrombectomy,demonstrating potential value for predicting poor clinical outcomes.关键词
