头颈部动静脉畸形/瘘介入栓塞联合手术治疗的疗效分析

Abstract
Objective:To investigate the clinical efficacy and safety of interventional embolization combined with sur-gical resection in the treatment of cervicofacial arteriovenous malformations/arteriovenous fistulas(AVM/AVF).Methods:A retrospective analysis was performed on 6 patients with cervicofacial AVM/AVF admitted from May 2018 to April 2024(4 males and 2 females,aged 28～65 years).All patients received the combined treatment of preoperative superselective emboli-zation plus surgical resection.Indicators including embolization materials(coils+NBCA glue),negative surgical margin rate,intraoperative blood loss,facial nerve preservation rate,and follow-up recurrence rate were recorded.Results:All 6 patients successfully completed the combined treatment.After embolization,the blood flow of the lesions decreased by 70％～90％,with a mean intraoperative blood loss of 320 mL(range,150～500 mL)and a 100％anatomical preservation rate of the facial nerve.The mean follow-up period was 48 months(range,18～84 months),with no recurrence.There were no injuries to the facial nerve or other cranial nerves,nor serious complications.Conclusion:Interventional embolization combined with surgical resection has the advantages of reducing surgical risks and preserving functions in the treatment of cervicofacial AVM/AVF.Multidisciplinary collaboration among interventional department,head and neck surgery department,and imaging department is the key to optimizing the therapeutic effect.关键词
