极端温度对中毒住院风险影响的病例交叉研究
A case-crossover analysis of the effect of extreme temperature exposure on poisoning hospitalization:a case study of Gansu Province

Abstract
Objective To explore the effect of extreme environmental temperature on the risk of hospitaliza-tion for poisoning and to provide a scientific basis for preventing and controlling poisoning in extreme weather.Methods With Gansu Province as the study area,poisoning hospitalization records and daily meteorological and air pollutant data for the same period were collected from 290 hospitals across the province from 2018 to 2022.Conditional logistic regression models and distributed lag nonlinear models were used to explore the nonlinear and lagged effects of ambient temperatures on hospitalization for poisoning cases.Results Ambient temperature was associated with the risk of hospitalization for poisoning in a U-shaped nonlinear relationship,with significant heterogeneity in the patterns of association across different types of poisoning.For example,alcohol poisoning was negatively correlated with temperature,carbon monoxide poisoning had a higher risk at-16 to 10 ℃,and pesticide poisoning was more pronounced at high temperature.Using the lowest risk temper-ature as a reference,extreme high temperatures(24.74 ℃,97.5th percentile)were associated with a signifi-cantly higher risk of hospitalization for poisoning(RR=1.58,95%CI:[1.47,1.70]).Similarly,extreme low temperatures(-12.80 ℃,2.5th percentile)were also linked to a significantly higher risk of hospitalization for poisoning(RR=1.20,95%CI:[1.08,1.34]).Subgroup analyses showed that the effects of extreme low tem-peratures were more pronounced in females,individuals over 60,those in the Central region,and patients hos-pitalized for carbon monoxide poisoning.In contrast,extreme high temperatures significantly affected males,younger adults under 18,residents of the Northwestern region,and hospitalization for pesticide and drug poi-soning.Conclusion In Gansu Province,the risk of hospitalization for poisoning was significantly associated with extreme temperatures,with a more pronounced heat effect,and this association exhibited distinct popula-tion and geographic characteristics.关键词
