山东临朐胃癌高发区主要癌症病例的住院费用分析
Analysis of inpatient costs for major cancers in Linqu County,Shandong Province of China
摘要
Abstract
Objective To assess temporal trends in per diem inpatient costs for major cancers and the cross-re-gional medical expenses based on Linqu County,Shandong Province,a high-risk area for gastric cancer.Methods A total of 1 074 lung,204 liver,657 gastric,450 colorectal and 131 esophageal cancer cases were enrolled through the Linqu Medical Insurance Information System.Medical expense records from 2021 to 2024 were derived for temporal trends analysis of per diem expense.Generalized linear model was employed to compare the disparities in the intra-county,extra-county and extra-provincial expense.Results From 2021 to 2024,the per diem expense significantly decreased for lung(P＜0.001)and gastric cancer(P=0.002),while it remained stable for liver(P=0.248),colorectal(P=0.311)and esophageal cancer(P=0.676).In 2021,the proportion of inpatient costs within the county accounted for 88.3％of total expenditures for major cancer cases while inpatients costs outside the county and outside the province accounted for 9.9％and 1.8％,respec-tively.By 2024,the proportion of inpatient costs within the county decreased to 61.1％,whereas those outside the county and outside the province increased to 28.4％and 10.5％,respectively.Overall,per diem inpatient costs outside the county and the province exceeded those for intra-county care.Conclusion From 2021 to 2024,per diem hospitalization costs for patients with liver cancer,colorectal cancer,and esophageal cancer in Linqu County remained stable overall,while costs for lung cancer and gastric cancer cases showed declining trends.The proportion of hospitalization expenses incurred by major cancer patients in Linqu County for care received outside the county but within Shandong province,and outside Shandong province,increased annual-ly during this period,and per diem hospitalization costs were higher than those for within-county patients.关键词
