Abstract

Objective To evaluate the clinical efficacy and safety of using a 1.0 mm diameter punch combined with mi-cro-plasma radiofrequency for the treatment of keloids.Methods A total of 73 patients with keloids treated between June 2021 and January 2024 were included.Among them,44 patients received combined punch and micro-plasma radiofrequency therapy(combined treatment group),while 29 were treated with micro-plasma radiofrequency alone(micro-plasma radiofrequency group).Both groups underwent four treatment sessions at 4-week intervals.The Vancouver Scar Scale(VSS)was used for assessment be-fore treatment and 6 months after the final session.High-frequency ultrasound was used to measure scar thickness and elasticity.Paired t-tests were used for within-group comparisons,and analysis of variance(ANOVA)was applied for between-group differ-ences.Results In the combined treatment group,scar thickness decreased from(4.88±1.17)mm to(2.39±1.16)mm(t=15.48,P<0.001),elasticity values improved from(31.45±9.66)kPa to(37.61±11.64)kPa(t=15.48,P<0.001),and VSS scores decreased from(10.14±1.03)points to(6.91±1.27)points(t=15.83,P<0.001).In the micro-plasma radiofre-quency group,scar thickness decreased from(3.78±0.67)mm to(3.09±0.96)mm(t=5.70,P<0.001),elasticity values im-proved from(28.15±7.45)kPa to(32.09±8.40)kPa(t=4.55,P<0.001),and VSS scores decreased from(8.79±1.34)points to(7.31±1.22)points(t=4.94,P<0.001).The combined treatment group showed significantly greater improvement compared to the micro-plasma radiofrequency group(P<0.001).The combined treatment group showed an effective rate of 93.18%,with a recurrence rate of 9.1%after6 months of follow-up.In the micro-plasma radiofrequency group,the effective rate was65.52%,with a recurrence rate of 20.69%.Conclusion The combination of small-diameter punch and micro-plasma radio-frequency demonstrates ideal clinical efficacy,high safety,and a low recurrence rate in the treatment of keloids,suggesting its po-tential for clinical practice.
