甲状腺手术中甲状旁腺非正力臂肱桡肌内自体移植的临床疗效
Clinical Efficacy of Autologous Parathyroid Gland Transplantation into the Brachioradialis Muscle with a Non-Opti-mal Lever Arm During Thyroid Surgery

Abstract
Objective To evaluate the outcomes of autologous transplantation of the parathyroid gland into the brachiora-dialis muscle with a non-optimal lever arm and analyze the temporal patterns of graft function.Methods From October 2022 to October 2023,total 52 patients who underwent thyroid surgery with concurrent autologous parathyroid gland transplantation into the brachioradialis muscle with a non-optimal lever arm in our hospital were selected.Serum PTH in the cephalic vein at the elbow on the transplant side and non-transplant side were analyzed on postoperative days 1,3,1 week,1 month,2 months,3 months,6 months,and 1 year.Patients'serum calcium within 24 hours post-surgery and the occurrence of hypoparathyroidism were also recor-ded.Results On the first postoperative day,PTH on both the transplant and non-transplant sides decreased with no significant difference(P>0.05).Statistical significance was observed in comparing the levels on both sides between postoperative week 1 and day 3,month 1 and week 1,and month 2 and month 1(P<0.05).There was significant difference between the transplant side at 6 months versus 3 months and 1 year versus 6 months(P<0.05).No significant differences were observed on both sides when comparing 3 months versus 2 months,the non-transplant side's 6 months versus 3 months,and 1 year versus 6 months(P>0.05).The PTH on the transplant side were significantly higher than on the non-transplant side from postoperative week 1 to 1 year,with statistical significance(P<0.05).The effective rates of grafts during the follow-up period from postoperative day 1 to 1 year were 1.9%,7.7%,40.4%,84.6%,84.6%,73.1%,69.2%,and 73.1%,respectively.Total 17.3%of patients experienced transient hypoparathyroidism post-surgery,with no cases of permanent hypoparathyroidism identified.Conclusion Autologous transplanta-tion of the parathyroid gland into the brachioradialis muscle with a non-optimal lever arm is a reliable method for preserving para-thyroid function.The graft begins to resume secretory function one month post-surgery,reaches optimal secretion state in 2-3 months,and gradually stabilizes with a decline in function post 6 months.关键词
