基于"水土合德"制方理念运用固精汤治疗早泄的辨析
Analysis of applying Gujing Tang based on the"harmony between water and earth"formula concept in treating premature ejaculation
摘要
Abstract
Premature ejaculation,a common male sexual dysfunction,is one of the main types of ejaculatory disor-ders.Western medicine has not yet reached a consensus on its pathological mechanism.Commonly used clinical treatments such as dapoxetine hydrochloride exhibit issues including unclear efficacy,significant adverse reactions,high recurrence rate,and drug dependence,resulting in unsatisfactory therapeutic effects.In contrast,traditional Chinese medicine demon-strates notable advantages in improving clinical symptoms,reducing adverse reactions,and promoting overall regulation.Professor Xu Fusong,a nationally renowned TCM expert,pioneered the concept of simultaneous treatment of the spleen and kidney in andrology,proposing that although the pathogenesis of andrological diseases involves the five zang-organs and six fu-organs,it primarily attributes to kidney dysfunction,with emphasis on the mutual root relationship between the spleen and kidney.His academic successor,Zhang Maosen,has deeply grasped this essence and innovatively integrated the TCM theory of"harmony between water and earth"to advance a new perspective for treating premature ejaculation by addressing the spleen and kidney.He formulated Gujing Tang(semen-securing decoction)for clinical application,achieving definite efficacy.Based on the core of the"harmony between water and earth"theory,combined with innovative doctrines such as"deficiency of the middle energizer leading to essence transformation"and the"spleen-kidney-genitalia axis,"this study thoroughly investigates the pattern identification and treatment strategies for premature ejaculation.With spleen-kidney defi-ciency as the key pathogenesis,which may secondarily lead to heart-kidney disharmony and failure of the liver to govern free flow,the fundamental approach involves tonifying the kidney and strengthening the spleen.This is complemented by securing essence to stop emission,calming the heart and soothing the liver,and nourishing the liver and kidney.Clinical cases are provided for verification,offering new insights and effective solutions for the clinical management of premature ejaculation.关键词
