晚期胃癌病人免疫治疗前后外周血淋巴细胞亚群变化及其临床意义
Dynamic changes of peripheral blood lymphocyte subsets before and after immunotherapy in patients with advanced gastric cancer and their clinical significance

Abstract
Objective To investigate the changes in peripheral blood lymphocyte subsets in patients with advanced gastric cancer before and after immunotherapy and their clinical significance.Methods A total of 190 patients with advanced gastric cancer who received at least four cycles of treatment with the immune checkpoint inhibitor sintilimab in Department of Oncology,Yantai Yuhuangding Hospital,from June 2022 to May 2024 were enrolled,and according to treatment response,they were divided into progression group with 29 patients and non-progression group with 161 patients.The levels of lymphocyte subsets before and after immunotherapy were compared between the two groups,and the influencing factors for progression-free survival(PFS)and overall survival(OS)were analyzed.Results After four cycles of treatment,the progression group had significant reductions in the levels of CD3+T cells,CD4+T cells,and CD8+T cells(t=2.735-4.703,P＜0.05)and a significant increase in the level of NK cells(t=3.969,P＜0.05).There was a significant difference in the baseline value of NK cells between the progression group and the non-progression group(t=2.109,P＜0.05).There were significant differences between the two groups in the changes in CD3+T cells,CD4+T cells,CD8+T cells,and NK cells before and after treatment(t=2.416-5.253,P＜0.05).The number of treatment lines and age were independent influencing factors for PFS(P＜0.05),and the percentages of CD8+T cells and NK cells at baseline and the expression of programmed death-ligand 1 were independent influencing factors for OS(P＜0.05).Conclusion Dynamic monitoring of lymphocyte characteristics before and after immunotherapy in patients with gastric cancer can help to for-mulate individualized treatment regimens and improve the efficacy of immunotherapy.关键词
