奥塔戈运动对首次脑梗死患者平衡能力及跌倒自我效能感的影响
Effects of the otago exercise programme on balance ability and fall self-efficacy in patients with first-ever cerebral infarc-tion

Abstract
Objective To investigate the effects of the Otago Exercise Programme(OEP)on balance ability,fall self-efficacy,activities of daily living(ADL),mobility,and serum neurobiological markers in patients with first-ever cerebral infarction.Methods Sixty patients with first-ever cerebral infarction admitted between June 2023 and June 2024 were randomly assigned to an observation group or a control group(n=30 each).During the study,3 patients in the observation group and 2 in the control group dropped out,resulting in 55 patients who completed the trial(observation group 27 patients,control group 28 patients).The control group received conventional rehabilitation and exercise training.In addition to conventional care,the observation group received a 4-month OEP intervention,which comprised individualized strength and balance training three times per week during hospitalization,followed by a home-based training program supervised via a WeChat group after discharge.Outcomes were assessed at baseline,2 months(pre-discharge),and 4 months(after 2 months of home training)using the Berg Balance Scale(BBS),a Pro-kin balance system for static posturography,the Modified Falls Efficacy Scale(MFES),the Barthel Index,the Timed Up and Go Test(TUGT),and serum levels of Glial Fibrillary Acidic Protein(GFAP)and Brain-Derived Neurotrophic Factor(BDNF).Results After the in-tervention,the observation group demonstrated significantly greater improvements in BBS scores,static balance parameters(move-ment perimeter and ellipse area),MFES scores,Barthel Index scores,and reduction in TUGT completion time compared to the control group(all P<0.05).Additionally,the observation group exhibited significantly lower serum GFAP levels and higher BDNF levels than the control group(all P<0.05).These beneficial effects were more pronounced with longer intervention duration.Conclusion The Otago Exercise Programme is a safe and effective intervention that can significantly improve balance ability,fall self-efficacy,ADL,and mobility in patients with first-ever cerebral infarction.The underlying mechanisms may involve reduced neurological dam-age and enhanced neurotrophic support,suggesting that OEP is a comprehensive and promising rehabilitation strategy.关键词
