老年康复医院人工气道护理现状调查
Investigation on the current status of artificial airway care in a geriatric rehabilitation hospital

Abstract
Objective To investigate the current status of artificial airway care in a geriatric rehabilitation hospital and to provide a reference for improving the quality of nursing practice.Methods A cross-sectional survey was conducted from August 17 to 19,2024,involving 215 patients with artificial airways(189 with tracheostomy and 26 with endotracheal intubation)from 22 clinical de-partments of a Grade Ⅲ A geriatric rehabilitation hospital.Data were collected on airway humidification methods,cuff management(pressure monitoring methods and values),and the implementation of subglottic suctioning.Results Patients with artificial airways accounted for 18.6%of all inpatients.The primary humidification methods included:oxygen-driven nebulization(88 cases,40.9%),continuous pump humidification(76 cases,35.3%),heated humidifier(33 cases,15.3%),intermittent drip humidifica-tion(17 cases,7.9%),and heat and moisture exchangers(1 case,0.5%).Regarding cuff management,among 206 patients requi-ring cuff management,only 22(10.7%)had normal cuff pressure,125(60.7%)had pressure below the normal range,14(6.8%)exceeded the normal range,and 45(21.8%)had no cuff inflation.Only 3 departments(13.6%)were equipped with cuff manome-ters;the remaining departments primarily used finger palpation for estimation.As for subglottic suctioning,171 out of 206 patients(83.0%)had a subglottic suctioning tube,among whom 105(61.4%)actually received subglottic suctioning.Although all 22 de-partments had the necessary equipment,none could ensure that all eligible patients received this intervention.Conclusion Signifi-cant deficiencies exist in artificial airway care within the surveyed geriatric rehabilitation hospital,particularly in the standardization of cuff pressure management,implementation rate of subglottic suctioning,and rational selection of humidification methods.There is an urgent need to develop evidence-based standardized nursing protocols,strengthen training and quality control for nursing staff,and improve care quality to ensure patient safety.关键词
