儿童骨性Ⅱ类错颌畸形修复中锥形束CT三维头影测量与传统X射线片测量方法的对比研究
Study on CBCT 3D cephalometry and conventional X-ray radiography measurement in orthodontic rehabilitation of skeletal class Ⅱ malocclusion of children

Abstract
Objective:To explore clinical value of three-dimensional(3D)measurement technique of cone-beam computed tomography(CBCT)compared with conventional X-ray cephalometry in the diagnosis for skeletal Class Ⅱ malocclusion of children.Methods:A retrospective collection was conducted on 100 pediatric children who were diagnosed as skeletal Class Ⅱ malocclusion at the Fourth Affiliated Hospital of Nanjing Medical University between January 2023 and June 2024.According to the measured method,they were divided into CBCT group(n=20)and X-ray radiography group(n=80).A 3D coordinate system was established to measure a series of conventional parameters,including the angle of sella-nasion-A point(SNA),sella-nasion-B point(SNB),the angle of A point-nasion-B point(ANB),and the angle of mandibular plane-sella nasion(MP-SN).Additionally,3D condylar displacement and pharyngeal airway volume were added.The SNA,SNB,ANB,MP-SN,condylar displacement,airway volume,clinical efficacy,measurement reliability,and dose of radiation exposure of two groups were observed and compared.Results:The measured value of ANB angle of CBCT group was significantly higher that of X-ray radiography group(t=3.217,P=0.002),and the value of MP-SN angle of CBCT group was significantly lower than that of X-ray radiography group(t=2.985,P=0.003).The condylar average distances of forward movement and upward movement of CBCT group were respectively(1.23±0.45)mm and(0.87±0.32)mm on x-axis and z-axis,while there was not information of 3D displacement in X-ray group.The airway volume of CBCT group was significantly lower than that of X-ray radiography group(t=5.217,P<0.001).The coordination degree of occlusal relationship,and degree of facial profile improvement of CBCT group were superior than those of the X-ray radiography group(x2=4.326,3.981,P<0.05).Intragroup correlation coefficients(ICCs)on angles of SNA,SNB,ANB,and MP-SN in CBCT group were all higher than those in X-ray radiography group,and the differences were significant(F=16.327,14.895,9.874 and 8.932,P<0.001).The effective dose and thyroid absorbed dose of CBCT group were significantly higher than those of X-ray radiography group(Z=8.927,9.123,P<0.001).Conclusion:CBCT-based 3D cephalometry has higher precision and clinical value in diagnosing skeletal class Ⅱ malocclusion of children,but it has higher radiation dose,which need individual risk-benefit assessment.关键词
