MRI脑白质病变量化评估与认知功能MMSE评分的相关性分析及影响因素探讨
Correlation analysis between quantitative MRI assessment of brain WML and MMSE scores of cognitive function,and exploration of influencing factors

Abstract
Objective:To investigate the correlation between quantitative assessment of magnetic resonance imaging(MRI)for brain white matter lesions(WML)and cognitive scores of Mini-Mental State Examination(MMSE),and to analyze factors that it affects cognitive function.Methods:A total of 105 patients who were diagnosed as WML by MRI at the Department of Neurology,Emergency General Hospital between January 2023 and December 2024 were enrolled.After other cerebral diseases and mental disorders were excluded,patients were divided into observation group with cognitive impairment(42 cases,MMSE<24)and control group without dysfunction(63 cases,MMSE≥24).Fazekas score and Fazekas scoring system were adopted to quantify the severity of lesion of T2-FLAIR sequences,and cognitive function was assessed by the MMSE.Multivariate logistic regression was used to correct a series of confounding factors included age,education years and hypertension.Results:The Fazekas score in the observation group was(2.69±0.63),which was significantly higher than(1.52±0.71)in the control group,with a statistically significant difference(t=5.841,P<0.05).In cardiovascular risk factors,the differences of low-density lipoprotein cholesterol(LDL-C),homocysteine,and fibrinogen between two groups were statistically significant(t=3.885,6.038,5.119,P<0.05).The difference of lesion volume between two groups was statistically significant(t=8.992,P<0.05).There were significant differences at C-reactive protein(CRP)and D-dimer of the indicators of laboratory between two groups(t=6.224,5.678,P<0.05).After 7 variables with statistical significance(age,education years,hypertension,LDL-C,homocysteine,Fazekas scores and CRP)in univariate analysis were included the model,the logistic regression indicated that Fazekas score and homocysteine were independent risk factors(OR=6.57,1.17,P<0.05),and education years was the protective factor(OR=0.77,P<0.05).The univariate analysis indicated that the correlations between hypertension and the risk of disease,and LDL-C and the risk of disease did not reach statistically significant difference(OR=2.86,1.20,P>0.05).Conclusion:WML is an independent risk factor for cognitive impairment,and the quantitative assessment of that exhibits a significant correlation with MMSE scores.The integrated comprehensive management of education,age and cardiovascular factor can contribute to optimize intervention strategies at early stage.关键词
