开窍通腑针法治疗急性缺血性脑卒中的临床研究
Clinical Study on the Kaiqiao Tongfu Acupuncture Method for the Treatment of Acute Ischemic Stroke

Abstract
Objective:To observe the clinical efficacy of the Kaiqiao Tongfu acupuncture method in patients with acute ischemic stroke and to explore its potential mechanisms of action.Methods:80 patients with acute ischemic stroke were randomly assigned by a random number table into an observation group and a control group,with 40 patients in each group.The control group received conventional western medical treatment combined with standard acupuncture,while the observation group received conventional western medical treatment combined with the Kaiqiao Tongfu acupuncture method.After 10 days of treatment,the following outcomes were observed and compared between the two groups:changes in National Institutes of Health Stroke Scale(NIHSS)scores,modified Rankin Scale(mRS)scores,Activities of Daily Living(ADL)scale scores,defecation symptom scale scores,and the differences(before vs.after treatment)in these scores.Serum levels of tumor necrosis factor-α(TNF-α),inter-leukin-6(IL-6),motilin,and vasoactive intestinal peptide(VIP)before and after treatment were compared.Tradi-tional Chinese medicine(TCM)syndrome scores were also evaluated.Results:After treatment,NIHSS and mRS scores were significantly decreased in both groups compared with before treatment levels(P＜0.05),while ADL scores were significantly increased(P＜0.05).In the observation group,defecation symptom scores were significant-ly reduced after treatment(P＜0.05).The differences in NIHSS,mRS,ADL,and defecation symptom scale scores(after treatment minus before treatment)were significantly greater in the observation group than in the control group(P＜0.05).In the observation group,serum levels of TNF-α was decreased,motilin,and VIP were increased significantly decreased after treatment(P＜0.05).The reductions(differences)in serum TNF-α was lower,motilin and VIP levels were significantly greater in the observation group than in the control group(P＜0.05).TCM syn-drome scores decreased significantly in both groups after treatment(P＜0.05),and the TCM syndrome score after treatment in the observation group was significantly lower than that in the control group(P＜0.05).Conclusion:The Kaiqiao Tongfu acupuncture method can improve neurological deficits,enhance activities of daily living,and reduce the risk of constipation in patients with acute ischemic stroke.Its mechanism may involve the suppression of inflammatory factors and the regulation of enteric peptides(motilin and vasoactive intestinal peptide).关键词
