改良V-Y皮瓣技术在肛周皮肤缺损修复中的临床应用分析
Clinical application analysis of modified V-Y Flap technique in repairing perianal skin defects

Abstract
Objectives To evaluate the clinical efficacy and technical aspects of a modified V-Y flap technique for repair-ing perianal skin defects,comparing it with the conventional V-Y flap technique.Methods Twenty-seven patients who un-derwent V-Y flap technique repair for perianal skin defects at Huangshan People's Hospital between March 2015 and February 2025 were included.They were divided into a conventional group(The conventional V-Y flap technique was ad-opted for treatment,n=11)and a modified group(The modified V-Y flap technique was adopted for treatment,n=16)based on the surgical technique used.Operative time,postoperative hospital stay,wound healing time,complications at 1 month(flap necrosis,wound dehiscence,infection,fat liquefaction,hematoma,anal stenosis/incontinence),anal function(Wexner incontinence score),and wound pain(Visual analogue scale,VAS)were compared.Results The modified group had significantly shorter operative time and postoperative hospital stay than the conventional group(P＜0.05).Wound healing time showed no significant difference(P＞0.05).Complications(wound dehiscence,fat liquefaction,infection)oc-curred in 7 patients in the conventional group versus 1 patient in the modified group with statistically significant differ-ence(P＜0.05).No cases of flap necrosis,hematoma,or anal stenosis/incontinence occurred in either group.There was no significant difference in Wexner incontinence scores between the two groups before,1 month after,3 months after and 6 months after surgery(P＞0.05).The Wexner incontinence scores at 1 month,3 months and 6 months after sur-gery in the two groups were higher than those before surgery(P＜0.05);The Wexner incontinence scores at 3 months after surgery was higher than that at 1 month after surgery(P＜0.05).The Wexner incontinence scores at 6 months af-ter surgery was lower than those at 1 month and 3 months after surgery(P＜0.05).The VAS scores of the modified group were lower than those of the traditional group on the 1st day,3rd day,7th day and 1 month after surgery(P＜0.05).The VAS scores on the 3rd day,7th day and 1 month after surgery in the two groups were lower than that on the 1st day after surgery,and the VAS scores on the 7th day after surgery was lower than that on the 3rd day after surgery(P＜0.05);The VAS scores at 1 month after surgery was lower than that on the 3rd day,7th day after surgery(P＜0.05).Conclusion The modified V-Y flap technique,featuring an arc-shaped apical angle design,non-sutured apex,and open drainage,effectively reduces postoperative complications and promotes wound healing,making it an ideal option for re-pairing complex perianal skin defects.关键词
