超微创钳辅助微单孔腹腔镜胆囊切除术的临床实践
Clinical application of ultra-minimally invasive forceps-assisted micro-single incision laparoscopic cholecystectomy

Abstract
Objective To analyze the feasibility and safety of ultra-minimally invasive forceps-assisted micro-single incision laparoscopic cholecystectomy(MSILC)in the treatment of benign gallbladder diseases.Methods Clinical data of 76 patients with benign gallbladder diseases,who admitted in Xuzhou Medical University Affiliated Xuzhou Municipal Hospital from October 2024 to June 2025,was retrospectively collected.All patients received MSILC with the assistance of ultra-minimally invasive forceps,and indocyanine green(ICG)fluorescence cholangiography was additionally used in 3 patients.Results All operations were successfully completed.Seventy-three patients required 1 ultra-minimally invasive forcep,two patients required 2 forceps,and one patient was converted to conventional three-port LC.No conversion to open surgery or no perioperative death occurred.The median operative time was 65(50,90)minutes,and median intraoperative blood loss was 10(10,20)mL.ICG fluorescence provided clear visualization of the bile duct structures.Twenty-eight patients required postoperative drainage due to severe inflammation or exudation,the median drainage duration was 48(40,80)hours.Median time to first ambulation after MSILC was 6(5,6)hours,and median postoperative hospital stay was 5(4,6)days.All incisions healed primarily with good recovery.No bile duct injury,intra-abdominal bleeding,bile leakage,umbilical hernia,postoperative pancreatitis,or pulmonary infection occurred during follow-up(1-8 months).Surgical scars were hidden and cosmetically satisfactory.Conclusion Ultra-minimally invasive forceps-assisted MSILC is safe and feasible in the treatment of benign gallbladder diseases.It minimizes abdominal wall trauma and provides excellent cosmetic outcomes.Intraoperative ICG fluorescence cholangiography enhances visualization of the Calot's triangle and biliary anatomy.关键词
