一例感染坏死性胰腺炎并发结肠出血的MDT诊治

Abstract
Pancreatic pseudocysts(PPC)and walled-off necrosis(WON)are local complications in the later stages(≥4 weeks)of acute pancreatitis(AP),which can be complicated by secondary infection and hemorrhage,leading to further complications such as infected pancreatic necrosis(IPN),intracystic hemorrhage and gastrointestinal bleeding.Due to the complex etiology,varied presentations,diagnostic challenges and high mortality,these conditions require rigorous clinical attention.This article presented a case of chronic pancreatitis with acute exacerbation,complicated successively by hemorrhage within pancreatic cysts,IPN and colonic hemorrhage.Despite multidisciplinary team(MDT)-guided conservative management and endoscopic intervention,the patient experienced recurrent bleeding and uncontrolled infection,ultimately necessitating emergency surgery(distal pancreatectomy+splenectomy+splenic flexure resection+partial gastrectomy+colonic anastomosis+ileostomy)under hemodynamically unstable conditions.The patient recovered well postoperatively.In conjunction with a literature review,this article focuses on the MDT collaborative approach,the selection of individualized treatment strategies,and the timing of surgical intervention for complex complications of pancreatitis,aiming to provide clinical reference.关键词
