清肺平喘汤治疗慢性阻塞性肺疾病急性加重期痰热郁肺证的临床研究
Study on Qingfei Pingchuan Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease with phlegm-heat stagnating in lung pattern

Abstract
Objective To explore the clinical efficacy and safety of the combined use of Qingfei Pingchuan Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with phlegm-heat obstructing the lung.Methods Retrospective selection of 50 AECOPD patients admitted to the Department of Pulmonary Disease at Taizhou Traditional Chinese Medicine Hospital from October 2022 to October 2023,who were treated with Qingfei Pingchuan Decoction in addition to conventional Western medicine treatment(combination treatment group).A 1:1 matching design was used to retrospectively screen 50 AECOPD patients who only received conventional Western medicine treatment(control group)based on core disease variables and basic variables.Both groups received continuous treatment for 10 days.The traditional Chinese medicine(TCM)syndrome scores,pulmonary function indexes,inflammatory response indexes,clinical efficacy and incidence of adverse reactions were compared between the two groups before and after treatment.Results After 10 days of treatment,the total TCM syndrome score,cough score,and wheezing score of the combined treatment group were lower than those of the control group,with statistically significant differences[(7.16±4.39)vs.(9.44±5.27),(1.00±1.16)vs.(1.92±1.66),and(0.92±1.16)vs.(1.56±1.42),respectively;t=2.351,3.207,2.474,P<0.05].After 10 days of treatment,the forced expiratory volume in 1 second(FEV1)of the combined treatment group was higher than that of the control group,and the difference was statistically significant[(60.74±8.34)L vs.(55.36±8.26)L;t=-3.240,P<0.05].After 5 days of treatment,the C-reactive protein(CRP)level of the combined treatment group was higher than that of the control group(39.52±15.91)mg/L vs.(32.54±10.57)mg/L;t=-2.584,P<0.05].After 10 days of treatment,the levels of white blood cells(WBC)count and CRP in the combined treatment group[(8.51±2.31)×109/L and(16.56±9.13)mg/L]were higher than those in the control group[(7.26±2.04)×109/L and(13.46±5.56)mg/L;t=-2.860 and-2.051,respectively,P<0.05].The total response rate of the combined treatment group was 98.0%(49/50),which was higher than 84.0%(42/50)in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Integrated therapy with Qingfei Pingchuan Decoction significantly enhances clinical efficacy and improves pulmonary function in patients with AECOPD presenting with the phlegm-heat obstructing the lung pattern,demonstrating a favorable safety profile.关键词
