Stem cells and their exosomes:novel therapeutic perspectives in treating lung disease

Abstract
Respiratory diseases are associated with high morbidity and mortality,posing a significant threat to global public health.Current clinical treatments primarily focus on relieving symptoms or slowing disease progression,but effective curative options yet remain unavailable.In recent years,stem cells and their derived exosomes have demonstrated considerable therapeutic potentials in pulmonary diseases,owing to their immu-nomodulatory,anti-inflammatory,anti-fibrotic and regenerative properties.As is known,stem cells exert their effects mainly through paracrine mechanisms,promoting lung tissue repair and regeneration.Notably,their exosomes,as essential mediators of intercellular communication,can independently deliver bioactive mole-cules and replicate many of the therapeutic effects of their parent cells.This review highlighted recent advanc-es in using stem cells and exosomes to treat pulmonary diseases,particularly focusing on mesenchymal stem cells and their derived exosomes in preclinical and clinical studies.It also outlined current challenges and future directions,providing a reference for the ongoing development and clinical translation of cell-based ther-apies in respiratory medicine.关键词
