The relationship between and diagnostic value of serum Nesfatin-1 and T cell immune index with cognitive function in Parkinson's patients

Abstract
Objective To investigate into the association and prognostic significance of serum Nesfatin-1 and T-cell immune parameters in cognitive impairment of Parkinson's disease(PD)patients.Methods From Sep-tember 2020 to December 2023,94 patients with PD were selected as the PD group,and 56 healthy volunteers who underwent physical examinations during the same period were selected as the control group.Patients in the PD group were divided into a cognitive impairment group(54 cases,<26 points)and a non-cognitive impairment group(40 cases,≥26 points)based on the Montreal Cognitive Assessment(MoCA)score.Accord-ing to the Hoehn-Yahr(H-Y)rating,they were further divided into an early stage group(n=31),middle stage group(n=32),and late stage group(n=31).Clinical data and venous blood samples were collected.Enzyme-linked immunosorbent assay and flow cytometry were used to detect and compare the levels of serum Nesfa-tin-1 and peripheral blood T lymphocyte subsets between the control and PD groups.Pearson analysis was used to analyze the correlation between Nesfatin-1,CD3+,CD4+/CD8+levels and MoCA scores in the PD group.Receiver operating characteristic(ROC)curve were applied to evaluate the predictive value of Nes-fatin-1,CD3+and CD4+/CD8+ratio for the diagnosis of PD.Kaplan-Meier method was used to plot survival curves,and survival analysis was performed on PD patients.Results In the PD cohort,concentrations of Nesfatin-1,CD3+and the CD4+/CD8+ratio were found to be significantly reduced compared to the control group(P<0.05).Similarly,PD patients with cognitive impairment exhibited lower levels of Nesfatin-1,CD3+,and CD4+/CD8+ratio than those without cognitive deficits(P<0.05).Across the H-Y staging,a pro-gressive decline in Nesfatin-1,CD3+,and CD4+/CD8+ratio was observed among PD patients,with statis-tically significant differences noted(P<0.05).Pearson correlation analysis indicated a positive association between Nesfatin-1,CD3+,CD4+/CD8+ratio and MoCA scores in PD patients(P<0.05).ROC curve analysis revealed that the AUC values for Nesfatin-1,CD3+,CD4+/CD8+ratio,and their combined use in diagnosing PD were 0.989,0.979,0.940,and 0.996,respectively,indicating a significant diagnostic potential(P<0.05).Survival curve analysis demonstrated that PD patients without cognitive impairment had a higher sur-vival rate than those with(P<0.05).Conclusion The reduced levels of serum Nesfatin-1,CD3+,and CD4+/CD8+ratio in individuals with PD are linked to cognitive dysfunction,and these biomarkers are of clinical di-agnostic significance for PD.关键词
