宫内刨削系统联合左炔诺孕酮宫内缓释系统治疗子宫内膜息肉的疗效分析
Clinical efficacy analysis of Intrauterine Bigatti Shaver(IBSⓇ)combined with Levonorgestrel-releasing intrauterine system in the treatment of endometrial polyps

Abstract
Objective To investigate the clinical efficacy of the Intrauterine Bigatti Shaver(IBSⓇ)combined with Levonorgestrel-releasing intrauterine system in the treatment of endometrial polyps.Methods The clini-cal data of 122 premenopausal patients who underwent Intrauterine Bigatti Shaver operation and were patho-logically diagnosed as having endometrial polyps in The First People's Hospital of Lanzhou from January 2021 to June 2023 were retrospectively analyzed.According to the actual clinical treatment plan,they were di-vided into a control and an experimental group.In the control group,66 cases underwent Intrauterine Bigatti Shaver(IBSⓇ),whereas the experimental group(56 cases)received a combined treatment of Intrauterine Big-atti Shaver(IBSⓇ)and Levonorgestrel-releasing intrauterine system.The changes in endometrial thickness,menstrual volume and recurrence rate before and after treatment were compared between the two groups,to explore whether operation in follicular phase and non-menstrual period outside the follicular phase was a risk factor for postoperative recurrence.Results Transvaginal ultrasound monitoring of endometrial thickness in the control group was(7.54±1.99),(8.14±1.76),(8.59±1.33)mm at 1 month,6 months and 1 year after operation;The endometrial thickness of the experimental group was(6.18±1.58),(6.33±1.47),(6.96±1.22)mm at 1 month,6 months and 1 year after operation,respectively,which was lower than that of the control group,and the dif-ference was statistically significant(P<0.05).The menstrual volume of the control group was(54.74±5.16),(52.09±5.73)mL at half a year and 1 year after operation,respectively,and that of the experimental group was(51.33±5.77),(45.06±4.92)mL at half a year and a year after operation,respectively,which was lower than that of the control group,and the difference was statistically significant(P<0.05).One year after operation,the overall recurrence rate of patients in the experimental group was lower than that of the control group,the difference being statistically significant(P<0.05).There was a difference in the recurrence rate between the two groups when the patients were operated in the follicular phase or in the non-menstrual period outside the follicular phase,but the difference was not statistically significant(P>0.05).A total of 122 patients with endo-metrial polyps were successfully operated via the Intrauterine Bigatti Shaver(IBSⓇ),the median operation time was 6 min,and no complications occurred during the operation.Conclusions Intrauterine Bigatti Shaver(IBSⓇ)combined with Levonorgestrel-releasing intrauterine system in the treatment of endometrial polyps can significantly reduce endometrial thickness,reduce menstrual volume,improve curative effect and effectively reduce the recurrence rate.The non-menstrual period outside the follicular phase is not a risk factor affecting postoperative recurrence of Intrauterine Bigatti Shaver(IBSⓇ).关键词
