Multidimensional Methodological Quality Evaluation and Comprehensive Analysis of Expert Consensus and Evidence-based Guidelines for Sarcopenia

Abstract
Objective:To comprehensively evaluate the methodological and reporting quality of existing sarcopenia diagnosis and treatment guidelines and expert consensus statements,and to summarize key recommendations,with the aim of providing the best evidence for the clinical management of sarcopenia.Methods:A systematic search was conducted in Chinese and English databas-es,guidelines,and relevant professional websites to identify clinical guidelines and expert consensus statements on sarcopenia pub-lished from inception to January 31,2025.Four reviewers independently performed consistency assessments,and the methodological and reporting quality of the documents were evaluated using the Appraisal of Guidelines for Research&Evaluation Ⅱ(AGREEⅡ)and the Reporting Items for Practice Guidelines in Healthcare(RIGHT)tools.Results:A total of 20 guidelines and expert con-sensus documents were included.Among these,2 were rated Grade A(recommended),12 were Grade B(recommended after modifi-cation),and 6 were Grade C(not recommended at present).It is recommended to adopt the updated diagnostic standards from the Asian Sarcopenia Working Group and European Working Group,with an emphasis on community screening.Treatment should follow the principle of combined nutritional and exercise interventions,and the role of multidisciplinary consultation in management should be strengthened.Future publications of sarcopenia-related guidelines or consensus statements should focus on training in methodo-logical rigor and standardized development processes,particularly in areas such as evidence chain construction,rationality of partici-pant composition,and update mechanism design.Conclusion:The overall quality of current sarcopenia guidelines and expert con-sensus statements still has room for improvement,and their recommendations require further refinement.Future guideline develop-ment should ensure methodological rigor,integrate the patient perspective,strengthen interdisciplinary collaboration,and expand di-verse evidence sources to enhance scientific validity and applicability.关键词
