Exploring the Treatment of Diabetic Retinopathy from Spleen Theory Based on"Spleen-Endoplasmic Reticulum Correlation"

Abstract
Diabetic retinopathy(DR)is one of the common diabetic microvascular complications with high prevalence and is one of the four leading blinding diseases.Modern studies have shown that endoplasmic reticulum stress(ERS)plays a key regula-tory role in the early neurodegeneration,neovascularization,and other pathological processes of DR by modulating molecular mech-anisms such as unfolded protein response(UPR),apoptosis,and inflammatory factor release.According to Chinese medicine,DR is a disease of deficiency of the spleen,with loss of transportation of the spleen,loss of glory and nourishment of the eye as the root cause,unfavorable transmission,phlegm and stagnation of phlegm,and damage to the eye network as the target cause.The team innovatively put forward the theory of"balance of transmission of the spleen",which suggests that an imbalance of transmission of the spleen is the core disease mechanism of DR.Based on the theory of"spleen-endoplasmic reticulum",the team innovatively proposed that endoplasmic reticulum stress is the microcosmic manifestation of spleen qi weakness,while the imbalance of essence and microtransmission is the macroscopic outward manifestation of ERS,and both of them constitute the core pathogenetic mecha-nism of DR through the"micro-macro"bi-directional regulation network.The two together constitute the core pathogenesis of DR through the"micro-macro"two-way regulatory network.Therefore,based on the"spleen-endoplasmic reticulum"corre-lation,this article reveals the specific mechanism of spleen-healthy traditional Chinese medicines in regulating signaling path-ways to improve the endoplasmic reticulum homeostasis,which provides a molecular biological basis for the interpretation of the scientific connotation of the theory of"balance of spleen function and transmission",and at the same time,it aims to provide a molecular biological basis for the clinical treatment of DR based on the theory of the spleen.At the same time,we hope to provide theoretical support for the clinical treatment of DR from the perspective of spleen by compounding and compounding,and the pre-cise use of drugs.关键词
