Abstract
Objective To investigate the relationship of serum microRNA(miR)-195-5p and miR-181c expressions with disease severity and prognosis in elderly patients with hypertensive intracerebral hemorrhage(HICH).Methods From April 2023 to April 2024,127 elderly HICH patients admitted to Baoding No.1 Hospital were included as the HICH group.The severity of illness of patients in the HICH group was assessed using the National Institutes of Health Stroke Scale(NIHSS)score,HICH patients were assigned to a mild group of 49 cases(NIHSS score≤4 points),a moderate group of 48 cases(NIHSS score 5-20 points),and a severe group of 30 cases(NIHSS score>20 points).A total of 127 healthy volunteers who underwent physical examinations at Baoding No.1 Hospital during the same period were selected as the control group.The patients in the HICH group were followed up 30 days after discharge.Based on Glasgow Outcome Scale(GOS)scores,they were categorized into a poor prognosis group(1-3 points)of 38 patients and a good prognosis group(4-5 points)of 89 patients.Real-time fluorescence quantitative polymerase chain reaction(qRT-PCR)was applied to detect the levels of serum miR-195-5p and miR-181c.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum miR-195-5p and miR-181c levels for the prognosis of elderly HICH patients.Multivariate Logistic regression was applied to analyze the factors affecting the prognosis of elderly HICH patients.Results Compared with the control group,the levels of serum miR-195-5p and miR-181c in the HICH group significantly reduced(t=16.427,17.999;P<0.05).Moreover,serum levels of miR-195-5p and miR-181c decreased progressively with increasing severity of HICH(F=24.618,31.244;P<0.05).Compared with the good prognosis group,serum miR-195-5p and miR-181c levels in the poor prognosis group significantly reduced,while bleeding volume,white blood cell count,and D-dimer level significantly increased(t=7.049,7.197,32.408,4.073,7.262;P<0.05).Multivariate Logistic regression analysis showed that bleeding volume,miR-195-5p,and miR-181c were factors affecting poor prognosis in elderly HICH patients(OR=2.624,0.844,0.625;P<0.05).Serum levels of miR-195-5p and miR-181c demonstrated predictive value for prognosis in elderly HICH patients,and their combined predictive capability of these biomarkers was superior to either biomarker alone(Z=3.216,2.301;P<0.05).Conclusion Serum levels of miR-195-5p and miR-181c decreased with increasing disease severity in elderly HICH patients.Both biomarkers demonstrated predictive value for prognosis,and their combined prediction was superior to that of either biomarker alone.关键词
