化浊健脾通络方对缺血性卒中合并糖尿病大鼠的影响及作用机制
Effect and mechanism of decoction for resolving turbidity,fortifying spleen and unblocking collaterals on ischemic stroke and diabetes mellitus in rats

Abstract
Objective It is to observe the effect of decoction for resolving turbidity,fortifying spleen and unblocking collaterals on blood glucose and neurological function in rats with ischemic stroke and type 2 mellitus diabetes,and to ex-plore its mechanism of action.Methods Thirty male SD rats were taken,10 of which were randomly selected as sham oper-ation group,the remaining 20 rats were fed with high-fat,high-sugar diet and given streptozotocin(STZ)by intraperitoneal injection to establish models of type 2 mellitus diabetes,at 7 days after successful modeling,the middle cerebral arteries of the rats were ligated to establish models of ischemic stroke.The successfully modeled rats were randomly divided into model group and group of decoction for resolving turbidity,fortifying spleen and unblocking collaterals.The group of decoction for resolving turbidity,fortifying spleen and unblocking collaterals was given decoction for resolving turbidity,fortifying spleen and unblocking collaterals 9 mL/kg by gavage,the model group and sham operation group were given equal volume of nor-mal saline by gavage,all continuously treated for 14 days.The neurological deficit scores,levels of fasting blood glucose and inflammatory factor[interleukin-1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),lipopolysac-charide(LPS),lipopolysaccharide-binding protein(LBP)]of the rats in each group were compared,the content of short-chain fatty acid in feces of the rats was measured by gas chromatography-mass spectrometry(GC-MS),the gut microbiota composition of the rats was detected by 16S rRNA sequencing,followed by diversity analysis.Results The neurological def-icit scores on the 14th day of gavage,and fasting blood glucose on the 7th and 14th day of gavage were significantly lower in the rats of group of decoction for resolving turbidity,fortifying spleen and unblocking collaterals than those before gavage and those of the model group at the same time points(all P<0.05);while the cerebral infarction volume ratio and serum levels of IL-1β,IL-6,TNF-α,LPS and LBP were significantly lower than those in the model group at the end of the last gavage(all P<0.05).The contents of acetate and butyrate in fecal of group of decoction for resolving turbidity,fortifying spleen and unblocking collaterals were significantly higher than those in the model group(all P<0.05).The fecal gut mi-crobiota diversity and structure of both the model group and group of decoction for resolving turbidity,fortifying spleen and unblocking collaterals differed significantly from the sham operation group.At the phylum level,there were no significant differences in relative abundances of Firmicutes and Bacteroidetes between the model group and group of decoction for resol-ving turbidity,fortifying spleen and unblocking collaterals(all P>0.05).At the genus level,the relative abundance of Akkermansia muciniphila in the group of decoction for resolving turbidity,fortifying spleen and unblocking collaterals was significantly higher than that in the model group(P<0.05),while the difference in the relative abundance of Bacteroides was not significant between the two groups(P>0.05).Conclusion Decoction for resolving turbidity,fortifying spleen and unblocking collaterals can improve nervous function and reduce blood glucose of rats with ischemic stroke and type 2 melli-tus diabetes,and its mechanism may be related to regulation of microbial imbalance,secretion of short-chain fatty acids and release of serum inflammatory substances.关键词
